have two form boxes for input to avoid formatting errors?

Although the instructions on this page are clear, I'm seeing a lot of requests where there is a space after the opening parenthesis. If not too difficult, it would be good to change this page so it has two input boxes, one for the composer and one for the title, and then it takes care of putting in the necessary Request: and parenthesis when creating the page, such as:

Request:$title ($composer)
Vaarky, unfortunately your suggestion to display two input boxes instead of one can't be implemented using just the InputBox extension. We'd have to resort to complementary solutions, as JavaScript or the SimpleForms extension. Maybe I can do it, will give it a try. —Carlos (talk) 19:43, 27 December 2013 (UTC)
Maybe a simpler example would help. It took me considerable head scratching before I realized this special page was trying to tell me not to include the brackets <> with the <title>. Are they needed in the first place? Richard Mix (talk) 22:22, 27 December 2013 (UTC)
